• I accept that this increases the risk of an allergic reaction or sensitivity to the colour products used, which could include itching, swelling, burning, rash, blistering or other symptoms.

  
  

• I confirm that Queen St Hair is not liable for any reaction, injury, or damage (including to skin, hair, or scalp) that may occur as a result of undergoing a colour service without a patch test.

  
  

• I release Queen St Hair and any freelance stylists working within the building (No.3 Queen Street, Uppingham, LE15 9QR) from all responsibility should any adverse effects occur.
